Instructions

• All fields in gray should be provided for you. If you need to change your personal information, please do so in the 'Review/Edit My Info' application. Review/Edit My Info.
• After enrolling, you will receive additional emails requesting information, to include a copy of your transcript, from Headquarters CAP.
• It may take up to two weeks for your enrollment to be finalized once Headquarters CAP has obtained all necessary information.
